 Hospice and palliative care is compassionate care for persons facing a life limiting illness or injury.
 
 Hospice is a team approach to expert medical care, pain management, emotional, and spiritual support for the whole person.
Support is provided to meet loved ones needs also.
 Hospice is designed to focus on caring, life and living, not curing.
Hospice services are provided in a person’s home, in skilled nursing facilities, assisted care and some board and care residential care facilities within the Orange County area.
 Hospice services are covered under Medicare, Medi-cal, and most private insurances, HMO’s and managed care carriers.

Palliative Care

 Palliative care extends the principles of hospice care that a person could benefit by receiving this care earlier in an illness and disease process.
 
 No therapies are excluded from consideration.
 
 Individual needs are assessed and treatment options are explored with the patient, family, physician and hospice team.

 The Hospice team develops a care plan that meets each patient’s individual needs in all areas of care, for symptom control and pain management, Psychosocial needs, emotional needs, and spiritual needs.
