What it isCase management provides real time help and assistance in the Orange County community for seniors. Case Management is the coordination of in-home services by a caring professional who assesses a seniors needs and recommends services to the senior and family. Follow-up and continuing care is provided by the case manager, at the seniors or family request. This ensures the needs of a senior who is living at home are met. Case managers arrange for services with other
health care professionals and services to meet the needs of seniors. (Example
of Services, home caregiver, transportation, chore services, adult day
care, assistive devices, personal emergency response system, home
delivered meals, mental health programs, nursing care provided by home
health services, physical therapy, provided by home health services.)
This is just an example of the many services a case manager can provide.
All information is confidential for seniors and families and is
shared only with the client and family in coordination with the
